HIV is a virus that breaks down immune cells and thus affects your immune system, which is why it is called in full the human immunodeficiency virus. If you have HIV and don't get it treated you can get very sick from normally harmless infections. Then you have AIDS (acquired immunodeficiency syndrome). It usually takes 8 to 10 years before someone gets AIDS after contracting HIV. In the Netherlands we fortunately hardly ever see AIDS these days, because we usually detect HIV in time with an STD test.

What are the symptoms of HIV?
What kind of symptoms does acute HIV cause? If you just contracted HIV a few weeks ago, you may experience flu-like symptoms such as fever, muscle aches and fatigue. You may also experience night sweats, swollen lymph nodes, skin rashes, diarrhea, headaches and sore throats. Of course, you can get such symptoms from many other diseases, but if you have recently had unsafe sex with someone in a high-risk group or with many different people, it is definitely a good idea to take an HIV test if you have such symptoms.

HIV is sexually transmitted through vaginal fluids and semen, but fortunately is not highly contagious that way. It is perhaps the least contagious STD known. HIV is much more easily transmitted through blood, which is why anal sex (where small wounds/tears are more likely to occur) and sex during menstruation are slightly more likely.
The greatest risk of HIV transmission is during childbirth; this is when blood from the mother easily comes into contact with blood from the child. All pregnant women are therefore preventively tested in the Netherlands and properly counseled if HIV is found.

If someone with HIV is properly treated then you can't get HIV from him/her anymore, including unprotected sex. If the amount of virus in the blood is not measurable (undetectable) then someone is not contagious. Read more about the n=n campaign here and here (undetectable = not infectious).
It is recommended that you get tested for HIV if:
- You have symptoms that could fit with HIV
- You have been warned by a sex partner that he or she has HIV
- You have shared a needle with someone who may have HIV
- you or your sex partner are from a high-risk group*
*These are the high-risk groups:
- men who have sex with men
- sex workers
- transgender
- people from areas where HIV is common (Eastern Europe, Sub-Saharan Africa, South and Southeast Asia, South and Central America, Caribbean)
- people with many alternating sex partners (3 or more every six months)
Notice! If in the past 72 hours high risk for HIV, do not wait do not wait, but contact the GGD (during office hours) or Emergency Department (in the evening or weekend) immediately. Perhaps PEP can still be given. This medication can prevent you from contracting HIV.
You can prevent getting HIV by:
- use condoms
- Use PrEP preventively
- to use PEP immediately after a risk

What's PrEP?
PrEP (pre-exposure prophylaxis) is virus-inhibiting medication that you can take around your sex moments or daily, so you are well protected during sex. Against HIV, PrEP works at least as well as condoms. Against other STDs, PrEP does not protect.
For people at greatly increased risk of contracting HIV, such as sex workers and men who have unsafe sex with men, this medication is a godsend. By taking PrEP at a time in your life when you are at high risk, you can avoid having to take HIV medications for life. For many PrEP users, this removes anxiety, allowing them to enjoy finer sex.

What is PEP?
PEP (post-exposure prophylaxis) is emergency medication you can get after you have been at high risk for HIV. You can think of it as a kind of morning after-pill against HIV. It is a 4-week course of virus inhibitors.
You can only get PEP through the GGD or in the hospital in the Emergency Room (ED). It is important that you start the cure as soon as possible after the risk! So contact the GGD or SEH immediately. Is the risk more than 72 hours ago? Then unfortunately there is no point in starting PEP anymore.
Together with the doctor/nurse, you will discuss what tests are needed during the appointment. An appointment lasts an average of 15 minutes.
An HIV test is always done on blood. During your appointment, a tube of blood will be drawn from your elbow crease. Do you faint easily? Then make sure you have eaten something and mention it at your appointment. You do not need to be sober for an HIV test.
There are different types of HIV tests you can take. At OneDayClinic, we have 3 of them:
- the 'normal' HIV test via the lab
This is a so-called 4th generation combo test, which looks for the presence of antibodies to HIV-1 and HIV-2** and the presence of p24 antigen (part of HIV-1, found in early infection). If necessary, the lab also deploys confirmatory testing. This 4th generation HIV test can often become positive as early as between 2 and 6 weeks after infection, but is not at its maximum reliability until 3 months after possible infection (99.9%). The result usually takes 1-2 business days and can be read on our website with your unique code and date of birth. - The 'normal' HIV test as a rapid test in the clinic.
This is also a 4th generation combo test, but as a test strip that can be read by the doctor/nurse after 20 minutes. It too is most reliable from 3 months after possible infection (99.9%). You can choose whether you want to be called with the results or wait in the waiting room for a while until the test is ready. - The 'early detection' HIV RNA test (unique in the Netherlands!).
This is an HIV rapid test using a different technique: a PCR test. This looks to see if there is RNA of HIV-1 present in the blood; this is genetic material of the virus. Shortly after possible infection, this is the most sensitive and reliable test you can do. Starting 14 days after risk, the HIV-1 RNA test has 96-100% reliability. The test takes 90 minutes, after which you will be called with the results. Do you want the results the same day? Then make sure your appointment is no later than 2 hours before closing time.
** HIV-2 is a rare variant of HIV. It occurs mainly in West Africa. All cases in Europe can still be traced directly to that region. Also, HIV-2 is less severe than the normal form of HIV.

Getting anonymously tested for HIV/AIDS

Treatment of HIV involves tablets that you have to take every day. You can only get these medications in the Netherlands in an HIV treatment center run by a specialist. All HIV treatment centers work together, so the quality of HIV care is equally good throughout the Netherlands.
The earlier you start treatment, the better. That way you avoid getting sick with HIV or being able to pass it on to others. Nowadays, treatment is so good that you are no longer contagious if you are properly treated, usually after only 3-6 months. Also, people with HIV today have a normal life expectancy. Unfortunately, HIV is not yet curable. So you cannot safely stop taking the medication and have to keep taking it for the rest of your life; that's how you stay healthy.

Where can I take an AIDS test?
HIV and AIDS are confused by many people. This is quite logical, since they have a lot to do with each other and are often mentioned in the news in the same breath. We will explain the difference: HIV is the virus that can cause the disease AIDS. Without HIV, you cannot have AIDS. And if you are infected, it can stay in your blood for life. So if you want to know if you might have AIDS, you have to look in your blood for traces of that virus by taking an HIV test. Only when people have HIV for a long time and are not treated can it lead to AIDS. In this, your immune system is weakened and you can easily get all kinds of infections. AIDS is then only diagnosed when an HIV test is also done and it is positive. So a real "AIDS test" does not exist.
